Many older first-time moms face a dilemma when it comes to babe No. 2. The clock is ticking louder than ever. But doctors advise waiting at least a yr and a half after giving nativity before conceiving over again.

This is the standard advice, based on multiple studies and public health guidelines. Only deciding when to attempt again tin be a difficult decision — weighing medical take a chance against infertility risk. Now there are some new data points to factor in. A paper published Mon in the periodical JAMA Internal Medicine analyzed medical records from nearly 150,000 Canadian pregnancies to tease out how a mother's historic period influences the effects of a shorter-than-recommended interval betwixt pregnancies.

For older moms in a bustle, the bad news is that the study adds evidence that conceiving within 12 months of a nativity does hateful heightened health risks for both mother and kid. But epidemiologist Laura Schummers, who led the enquiry while at Harvard and is now a postal service-doctoral fellow at the University of British Columbia, says there'due south good news for you here likewise:

"The optimal spacing window that we found was one to ii years after the delivery of one child until the conception of the next pregnancy," she says. "In fact," Schummers says, "nosotros found that there were risks of agin infant outcomes for women of all ages.

"The risks to the babies were higher amongst younger women, which was consistent with the team's hypothesis. Just risks to the mothers were higher amidst older women — indeed, simply older mothers incurred higher risks to their own health by getting pregnant once more so soon.

Later on accounting for other factors that could drive these numbers, Schummers says, the stats shake out similar this:

• For women 35 years or older who conceived but six months after a birth, 6.2 per 1000 experienced serious disease or injury, including death. Wait 18 months and that risk dropped to 2.6 per per grand. So, small absolute numbers but a dramatic difference.

• A "severe adverse infant event" includes stillbirth and being built-in very early on or very pocket-size. Among women ages twenty to 34, those who conceived after just six months had xx babies per k with those astringent outcomes; the chance drops to 14 per k among those who waited eighteen months.

It seems like mutual sense that a woman'southward trunk may need more than six months to fully recover from building a babe and giving nascency, but the actual mechanism behind the risks of brusk pregnancy intervals is not fully clear.

The leading theory, Schummers says, is that nutrients like iron or folate could be depleted in the mother's body. But more than inquiry is needed to come across if that theory holds in adult countries like the United States and Canada, or if there are other mechanisms that accept not yet been identified.

For now, she says, her team hopes these new findings tin can help women make decisions within their own personal contexts, and in consultation with their medical teams. The data may be particularly helpful for older women, she says, because they more than frequently make up one's mind to accept brusque pregnancy intervals on purpose.

"And so if you're making that kind of decision on purpose," she says, "it'southward easier to say, 'You know, let's wait another three months.' "
